3-D Conformal Radiotherapy of Early Breast Cancer

Abstract
|
|
|
Purpose: This is a study to evaluate the dosimetric advantage of 3-D conformal radiotherapy plan in the treatment of early breast cancer. compared to 2-D conventional plan.
Materials and Methods: Six patients were enrolled and all patients underwent breast conserving surgery and were treated by 6 MV photon irradiation using a half beam tangential technique. Two plans were generated for each patient. The 2-D radiotherapy plan utilized a single contour taken at the central axis and 3-D radiotherapy plan was generated by using multiple CT contours. Dose-volume histograms(DVHs) were made for both plans and used for subsequent comparison. In 3-D radiotherapy plan, Y(logitudinal)-wedge was used in addition to X(axial)-wedge as a dose optimization tool.
Results: Irrespective of breast size and field separation, the better planning target volume(PTV) coverage by the 100% isodose curve was observed in every 3-D conformal radiotherapy plan. Especially in the upper part of breast with smaller amount of intervening lung volume, the benefit of 3-D plan was outstanding. The maximum doses in the two plans were almost same. But dose homogeneity index in 3-D radiotherapy plan deteriorated and this difference translated into statistical boderline significance. In the comparison of DVH of ipsilateral lung, no meaningful difference was noticed between the two plans.
Conclusion: 3-D conformal radiotherapy plan manifested a obvious advantage of the better target coverage but there was a trend to lower the dose homogeneity.
